What differrence between "made of" and "made from"?

"This table is made of wood." "Bread is made from wheat" What difference between them?

+ We use made from when the materials used to make it have been completely changed and can't be recognized. Ex: Bread is made from wheat. + We use made of when the materials used to make it haven't been completely changed and can still be recognized. Ex: This table is made of wood.
