What is the definition for "community dwelling elderly"?

Hello. I am translating a medical text from English to another language. I faced this word and need somebody to help me: "community dwelling elderly". What kind of elderly is this!!? Do other elderlies not live in the community? So who are community dwellings?

Community dwelling elderly = old people ( = elderly) that live ( = dwell) in a community
